What is the Transaxillary Endoscopic Breast Implant Surgery?
As the name suggests, in short, the transaxillary endoscopic technique is the placement of implants in the armpit using an endoscope. Breast implants can be placed either under the mammary gland (subglandular) or under the pectoral muscle (submuscular) through a minimal incision of about 3-4 cm. This incision is strategically positioned in a natural crease at the tip of the armpit, known as the transaxillary approach, to minimize the visibility of scarring and allow for faster recovery.
For precise implant placement, an endoscope, a thin instrument equipped with a camera and a light, connected to a video monitor, is used. It transmits high-definition 3D images of the internal structures, allowing the surgeon to clearly visualize the operative field and perform the dissection with maximum precision and clarity.
Breast Implant by Transaxillary Endoscopic Technique - Advantages over the Classic Technique
1. Absence of breast scars
The great and one of the most important advantages of transaxillary endoscopic breast implant surgery over the traditional technique is the absence of scars on the breasts. Why is this important?
The scars, once formed, are permanent; they cannot be removed with an eraser. By any method, laser or otherwise, scars cannot be completely removed. What's more, by trying to remove one scar, you run the risk of a new, perhaps even more annoying one appearing. The image below shows the appearance of scars 3 years after the traditional sub-mammary breast implant procedure.
In addition, scars from the classic technique stand out when you lie on your back, easily visible to your partner or when the patient wants to go topless.
And here's a case 10 years after a transaxillary endoscopic breast implant surgery, where the scar under the arm is virtually invisible.
2. No risk of scar opening
Because there is no scarring in the breast after underarm breast implant surgery, we virtually completely eliminate the risk of the scar coming undone, as there is no pressure on it.
This does not apply in the classic technique. With the under-breast breast implant method, there are situations (as in the case below) where the scar under the breast is loosened due to the tension of the breast implant.
3. Faster recovery and easier healing
Another great advantage of the transaxillary endoscopic technique is that the scar heals much better in a fold located in the armpit. In this area, the skin is as mobile as in the eyelids, and there is lax connective tissue which ensures much better healing than in other areas.
Scars under the breasts after traditional surgery are more difficult to heal; quite often, hypertrophic or even keloid scars appear.
There have been situations where patients, although they had keloid scars in other areas of the body, such as those resulting from caesarean section, after underarm breast implant surgery, the scar healed normally without any problems.

4. Significantly reduced risk of nipple sensitivity loss
Another equally important aspect is that the risk of loss of nipple and skin sensitivity from the nipple down to the inframammary fossa is much lower with the endoscopic axillary technique than with the classic submammary technique.
This is possible because the procedure is performed remotely, using a minimally-invasive technique and ultra-modern medical equipment that offers maximum precision.

5. Long-term stability of the intervention
Breast implant surgery using the transaxillary endoscopic technique does not sever the inframammary groove ligament. This is particularly important, as has been shown in numerous specialized studies. The inframammary ligament plays an essential role in maintaining the long-term stability of the breast implant.

6. You can wear any type of bra
You probably already know that, following a traditional breast implant operation, you should wear a certain type of bustier for a good while after the operation, but under no circumstances a bra with padding to avoid pressure on the scars.
Following the transaxillary endoscopic breast implant surgery, you can wear any type of bra, including those with "whale" / "whale" bra cups, because, having no scars under the breasts, it will not create any problems.
7. The implant can be placed either in front of or behind the pectoral muscle
Using the transaxillary endoscopic technique, the breast implant can be positioned both in front of and behind the pectoralis muscle, which sometimes allows us to perform an internal breast lift in the same operation. The prerequisite for this is that the degree of breast ptosis is small. In this way we are able to lift the breasts without those extra scars on the breasts.

Why is the Transaxillary Endoscopic Technique not so well known in Romania?
The transaxillary endoscopic technique can achieve remarkable results, but it is not an easy technique to perform, requiring complex expertise and special training on the part of the surgeon who performs it. Hence the relatively small number of doctors practicing this technique. Less than 1% of plastic surgeons worldwide practice underarm breast implants.
